INDIANA HEALTH CENTERS PAYMENT FOR SERVICES

Our clinic has a variety of health centers throughout Indiana including Logansport, Kokomo, Marion, Peru, South Bend and Seymour. They provide quality, affordable, primary health care services regardless of patients’ ability to pay.  Our uninsured patients are provided with an income-based sliding fee discount.  Additionally, we accept Medicare, Medicaid, and most commercial insurance plans.   Our staff members are trained and certified to assist patients with Medicaid applications.  We will also assist patients with signing up for commercial insurance products that are available through the Health Exchange Networks.

Medicare

We accept traditional Medicare as well as all commercial Medicare plans. To ensure proper patient identification, eligibility, and demographic information, patients should bring their Medicare card to each visit.

Medicaid

If you are Medicaid eligible and not already enrolled, our staff will assist you with completing an online Medicaid application at the time of your office visit.

Private Insurance

We accept most private insurance plans. If you qualify for a sliding fee scale discount, it will be applied to any deductible or co-pay.  To ensure proper patient identification, eligibility, and demographic information, patients should bring their insurance card to each visit.
