INDIANA HEALTH CENTER CASE MANAGEMENT

At IHC, we are a Federally Qualified Health Center that ensures that our patients have access to the medical services they need. Our case managers provide assistance to patients and their families by coordinating their referrals for health service needs. Case managers also integrate services with community agencies to address other needs our patients may have, including:

• Making appointments
• Coordinating patient care
• Answering service and financial questions and providing advice
• Linking with community resources
• Crisis intervention
