Family Nurse Practitioner

Jay Hoeksema, MSN APRN, FNP-C completed his Master of Science in Nursing at Keiser University Graduate School of Nursing and is licensed in the state of Indiana as an Advanced Practice Registered Nurse. Jay is board certified as a Family Nurse Practitioner through the American Nurses Credentialing Center (ANCC). He completed his Bachelor of Science in Nursing at Indiana University Kokomo in Kokomo, Indiana. Jay is certified in Advanced Cardiac Life Support (ACLS), Pediatric Advanced Life Support (PALS), and Basic Life Support (BLS) through the American Heart Association.

Joanna L. Moore, MSN APRN, FNP-BC

Family Nurse Practitioner

Joanna L. Moore, APRN, FNP-BC completed her Master of Science in Nursing at Indiana University Graduate School of Nursing in Kokomo, Indiana and is licensed in the state of Indiana as an Advanced Practice Registered Nurse. Joanna is board certified as a Family Nurse Practitioner through the American Nurses Credentialing Center (ANCC). She completed her Bachelor of Science in Nursing at Indiana University in Kokomo, Indiana. Joanna is certified in Basic Life Support (BLS) through the American Heart Association and holds memberships to the American Nurses Association and the Indiana State Nurses Association. Joanna has a passion for being a nurse and plans to cultivate that in others as a preceptor and mentor to new advanced nursing professionals.

Dr. Philip Renfroe, MD, MScPH

Chief Medical Officer

Dr. Philip Renfroe, MD, MScPH completed his family medicine training and internship at the Medical Center Department of Family Practice in Columbus, Georgia. Dr. Renfroe is board certified in Family Medicine with the American Board of Family Medicine (ABFM). Dr. Renfroe is a graduate of the Louisiana State University School of Medicine in Shreveport, Louisiana, where he completed his doctorate in medicine. Dr. Renfroe has earned a Master of Science in Public Health (Health Promotion) (MScPH) from the University of London in London, England and a Postgraduate Diploma of Public Health from the London School of Hygiene and Tropical Medicine in London, England. Dr. Renfroe served for many years as the Chief of Medical Staff and staff physician for adult, pediatric, and obstetrical services at Tenwek Hospital in Bomet, Kenya, East Africa. He also currently serves as the Assistant Professor of Public Health at Indiana Wesleyan University in Marion, Indiana.
