All Indiana Health Centers sites are PCMH recognized!

What does that mean? Patient-Centered Medical Home (PCMH) is a model of care delivery that focuses on coordinated, effective, and personalized care.

Each patient has an ongoing relationship with a personal provider who works with a team of health professionals, including nurses, pharmacists and specialists, to ensure that everyone is on the same page when it comes to their health. This also includes specialty care, hospitals, home health care, community services, and other support organizations.

This partnership between providers, patients, and their families ensures that patients have the education and support they need to make decisions and participate in their own care.
