Shannon Bates, Practice Manager
Shannon has over 20 years of experience in healthcare administration, and has been a practice manager for 14 years. Shannon has been with IHC since 2009 and currently serves as Practice Manager at both Cass Community Health Center in Logansport and Community Health Center of Miami County in Peru.
Carol Eldridge, MSN, FNP-C
Carol Eldridge, MSN, FNP-C completed her graduate training at Indiana University School of Nursing in Indianapolis, Indiana and is licensed in the state of Indiana as an Advanced Practice Nurse. Carol is board certified as a Family Nurse Practitioner through the American Academy of Nurse Practitioners (AANP). Carol also completed both her Bachelor of Science in Nursing and her Associate of Science in Nursing at the Indiana University School of Nursing. She is certified in Advanced Cardiovascular Life Support (ACLS), Pediatric Advanced Life Support (PALS), and Cardio Pulmonary Resuscitation (CPR).
Bryan Army, MSN, FNP-C
Bryan Army, MSN, FNP-C completed his training at the Indiana University-Purdue University Indianapolis Graduate School of Nursing with honors and is licensed in the state of Indiana as an Advanced Practice Nurse. Bryan is board certified as a Family Nurse Practitioner through the American Academy of Nurse Practitioners (AANP). He is an active member of the Emergency Nurses Association (ENA). He completed his Bachelor of Science in Nursing at Indiana University Kokomo School of Nursing in Kokomo, Indiana. Bryan was born and raised in Cass County and is a graduate of Lewis Cass High School. He is certified in Basic Life Support (BLS), Pediatric Advanced Life Support (PALS), and Advanced Cardiovascular Life Support (ACLS).
Jo Estes, MSN, FNP-C
Jo Estes, MSN, FNP-C completed her training at Indiana University Graduate School of Nursing in Indianapolis, Indiana and is licensed in the state of Indiana as an Advanced Practice Nurse. Jo is certified as a Family Nurse Practitioner through the American Academy of Nurse Practitioners (AANP). Jo is very active professionally and in her community. She has numerous professional affiliations including the ISNA Membership Chairman, ISNA Delegate to State Convention, ISNA District 11 Director, and Miami County Board of Health member. Jo is certified in Basic Life Support (BLS).
Dr. Hammad Liaquat completed his internal medicine residency training and his geriatric fellowship training at Indiana University Health in Indianapolis, Indiana. Dr. Liaquat is board certified in Internal Medicine and Geriatric Medicine. He is also a Diplomat of the American Board of Internal Medicine (ABIM). Dr. Liaquat is a graduate of Aga Khan University Hospital in Karachi, Pakistan where he completed his medical education. Dr. Liaquat is a member of both the American College of Physicians, American Society for Gastrointestinal Endoscopy, and the American Geriatrics Society.
Olga Ochoa Reina
Dr. Olga Ochoa Reina completed her pediatric residency training at the University of Illinois at Chicago Pediatric Residency Program located in Chicago, Illinois and is board certified in Pediatrics. Dr. Ochoa is a fellow of the American Academy of Pediatrics (AAP) and a Diplomate of Epidemiology. Dr. Ochoa is a graduate of Universidad Pontificia Bolivariana in Medellin, Antioquia, Colombia where she completed her medical education and internship. Dr. Ochoa is fluent in both English and Spanish. She is certified in both Pediatric Advanced Life Support (PALS) and Neonatal Resuscitation Program (NRP). Dr. Ochoa is an experienced pediatrician who spent more than ten years caring for minority and low-income populations in hospital and clinical settings as a physician prior to coming to the United States.
